We will guide you through through the process, provide the intake forms, and answer your questions regarding services.
Step 2: Submit Completed Forms
Email your forms.
Once you have completed all the forms, please email them to tina@ctxservicetribe.com. Please include a photocopy of your drivers license or state ID card and court order.
Set up an appointment.
We will schedule an intake appointment to review specific details of your situation.
